A 45-year-old man presents with severe central chest pain, nausea, and sweating for 30 minutes. No other symptoms are present. He was diagnosed with hypertension 10 years ago. This is well controlled on exercise and diet alone. His medical and surgical histories are unremarkable otherwise. A routine health checkup two months ago was normal; this included a fasting blood sugar, lipid profile, and an ECG. He is not on any medications. He only drinks socially, around 2 to 3 standard drinks each week. He does not smoke or use recreational drugs.
