A 32-year-old male is brought to the emergency department by paramedics after being found unresponsive in his bed by his partner. He has a 20-year history of type 1 diabetes mellitus. His partner reports that he went to bed appearing well but was difficult to rouse this morning and was noted to be “sweaty and twitching.” There is no history of fever, trauma, or recent illness. His current medications include insulin glargine and insulin lispro. He has a history of strict glycemic control with a most recent hemoglobin A1c of 6.2%. His partner notes he has had several episodes recently where he acted confused but denied feeling low blood sugar.
